Manuellterapi i kombinasjon med treningsterapi i behandling av coxartrose. "Kan smerte og funksjon påvirkes hos to pasienter?"

Background: There are evidence that both traction mobilization and exercise therapy each has impact on the treatment of osteoarthritis of the hip. However few studies are undertaken combining the methods. In this study we investigate if standard traction mobilization in combination with training therapy could impact on function and pain for two patients with unilateral osteoarthritis of the hip. Objective: «Could manual traction mobilization of the hip joint combined with training therapy have impact on functionality and pain for two patients with unilateral osteoarthritis of the hip?» Materials and method: In this study single Subject Experimental Design (SSED) was utilized. Two participants were included. Manual traction mobilization and exercise therapy were given. The study lasted 11 weeks, with 1 week baseline period, 6 weeks intervention period and 4 weeks control period. In the intervention period, 2 treatments per week were given, totalling 10 treatments. The outcomes were Hip Disability and Osteoarthritis Outcome Score (HOOS) for self reported functionality, Pain Numeric Rating Scale (PNRS) for pain and "Get Up and Go" for function. Results: After 6 weeks of intervention, function was improved in HOOS compared to the baseline period. PNRS showed improvement for pain and "Get Up and Go" showed improvement for walking ability. But the improvements in PNRS and "Get Up and Go" were too small to be clinically important. One month after the intervention was finished the improvement were maintained. Conclusions: The results indicate that the combination of traction mobilization and training therapy had impact on self reported functionality for both patients. The treatment also appeared to have impact on pain reduction and walking ability.Bakgrunn: Det foreligger dokumentasjon på at både traksjonsmobilisering og treningsterapi hver for seg har effekt på behandling av coxartrose. Få studier er gjennomført hvor disse to metodene kombineres i behandlingen av coxartrose. I denne studien belyses det om traksjonsmobilisering i kombinasjon med treningsterapi kan påvirke funksjon og smerter hos to pasienter med unilateral coxartrose. Problemstilling: "Kan manuell traksjonsmobilisering av hofteleddet kombinert med treningsterapi påvirke smerte og funksjon hos to pasienter med unilateral coxartrose?" Materiale og metode: I denne studien ble single Subject Experimental Design (SSED) benyttet. To deltakere ble inkludert. Manuell traksjonsmobilisering og treningsterapi ble benyttet som intervensjon. Studien varte i totalt 11 uker, med 1 uke baselineperiode, 6 uker intervensjonsperiode og 4 uker kontrollmålingsperiode. I intervensjonsfasen ble det gjennomført 2 behandlinger per uke, totalt 10 behandlinger. De måleinstrumentene som ble benyttet var Hip Disability and Osteoarthritis Outcome Score (HOOS) for selvrapportert funksjon, Pain Numeric Rating Scale (PNRS) for smerte og "Get Up and Go" for funksjon. Resultater: Etter 6 ukers intervensjon ble det registrert en bedring av selvrapportert funksjon i HOOS funksjonsskjema sammenlignet med baselineperioden. PNRS viste bedring i forhold til nedgang i smerte og "Get Up and Go" viste bedring i forhold til gangfunksjon. Men bedringen i PNRS og "Get Up and Go" var for liten til at den er klinisk betydningsfull. En måned etter avsluttet intervensjon var forbedringen opprettholdt. Konklusjon: Resultatene indikerer at kombinasjonen av traksjonsmobilisering og treningsterapi hadde effekt på selvrapportert funksjon for begge pasientene.
